Title and author name

Do have to have the title of the book and author name at the top of each page? If not, how do I remove it from the template?

No you do not need it.  You can delete it by  learning how to use the software you are using.  Try Googling something "remove header MS-Word", without the quotes.  I am not a patient enough person to give you instructions to do something like this.

Not only do you not need it, but most books I've seen don't have it (it used to be more common many decades ago.)  After all, of you're reading "My Adventures" by John Doe, do you really need to be reminded that that's the book you're reading every time you turn the page?  And if you forget what book you're reading, can't you just look at the cover?

A header maes more sense if you have a collection of short stories with different titles/authors, or perhaps different chapter titles.  For example the book Vegetable Gardening miight have the book title on one page and the name of the particular vegetable on the facing page.
